If you would like to register by mail or fax, please click here to download the registration form. The Advance Registration and Hotel Reservation Form consists of four pages labeled A, B, C and D as follows:
A. Contact Information: This section must be filled out by all attendees. Should you require housing, please fill out the Hotel Reservation section of this page indicating your hotel choice. A deposit equal to one night's room and tax charges at your selected hotel is required. Deposits will be refunded if reservations are cancelled 72 hours prior to arrival. B. ACC.08 ONLY Registration: Fill out this page if you would like to attend only ACC.08. Registration includes access to ACC.08 Education and the ACC.08 Exposition.
C. SCAI-ACCi2 ONLY Registration: Fill out this page if you would like to attend only The SCAI Annual Scientific Sessions in Partnership with ACC i2 Summit. Registration includes access to SCAI-ACCi2 Education and the ACC.08 Exposition.
D. ACC.08 and SCAI-ACCi2 Registration: Fill out this page if you would like to attend both ACC.08 and The SCAI Annual Scientific Sessions in Partnership with ACC i2 Summit. Registration includes access to ACC.08 Education, SCAI-ACCi2 Education, and the ACC.08 Exposition.

Name/Mailing Address. Address changes will be used for meeting materials only. Permanent address changes must be made with the ACC Resource Center at (800) 253-4636, ext. 5603, outside the United States, call (202) 375-6000, ext. 5603.
Badge Information. Indicate preferred name or nickname and degree for badge. The information will appear on the badge exactly as written. Due to space limitations, please list only one degree.
Family Member. Spouses and other family members 12 years and older must register at a fee of $50 to receive a family member badge, which allows entry into the Exposition and Poster sessions. Family member registrations must be accompanied by a professional registration. Note: Professionals who wish to attend education sessions and/or receive CME credit must register in a professional category.
Registration Profile. Please complete this section online and/or on the registration form. Without this information, your registration cannot be processed.
Special Needs. Please indicate special needs (hotel accommodation, transportation, wheelchair, etc.).

ACC or SCAI International Associate. International Associate members
ACC Cardiac Care Associate Member. Registered Nurse, Nurse Practitioner, Clinical Nurse Specialist, Physician Assistant and Pharmacist members
ACC or SCAI Affiliate Member Fellow in Training. Physicians who are members of the ACC through their enrollment in an accredited cardiovascular training program or members of SCAI through their enrollment in an accredited interventional training program
ACC or SCAI Scientific Session Abstract Presenter. Any attendee who will be attending and presenting an abstract (oral or poster)
Nonmember Physician. M.D., D.O., Ph.D.
Nonmember Trainee/Resident (verification required). Physician enrolled in an accredited training program but not a member of ACC or SCAI.
Nonmember Nonphysician (verification required). Nonmember registration category includes the following groups: nurse practitioner, nurse, physician assistant, clinical nurse specialist, nutritionist, pharmacist, research technician, exercise physiologist, technologist and any other nonphysician health care professional who deals with direct patient care.
Administrator. Individual responsible for the management of cardiovascular specialist practices. Qualifying individuals include clinic administrator, personnel administrator, and hospital administrator.
Nonmedical (no CME credit). Individual directly employed by a health care institution, network, corporate health care provider or company whose primary job function is ancillary to direct health care delivery. Qualifying individuals include chief executive officer, chief financial officer, chief information officer, personnel involved in commercial research and development related to cardiology, employee of an ACC.08 exhibitor, health care consultant, computer analyst, purchasing agent or consultant, corporate or executive manager.
Exhibit-Only (no CME credit). Fee admits registrants to Exposition Sunday, Monday and Tuesday. To qualify for this category, individuals must be eligible to register in a professional category.
One Day ACC.08. Fee admits registrants to the ACC.08 Exposition and any ACC.08 education sessions held that day.
One Day SCAI-ACCi2. Fee admits registrants to the ACC.08 Exposition and any SCAI-ACCi2 education sessions held that day.
Payment
The registrant MUST provide payment at the time of registration. Registration fees will be charged to the credit card provided immediately. Hotel deposits will be charged if reservations are not cancelled 72 hours prior to arrival. Wire transfers will not be accepted.
Two payments are required during the registration process; one for registration and one for housing. The registration payment will be charged immediately to your card. The hotel deposit will be charged to your card directly by the hotel.
